- Eye & Dental Certificate.
- Physical Fitness Certificate with Blood Group.
- 3 Passport Size photos.
- SC/ST & BC/MBC candidates must open Bank A/c number with IFSC and MICR Code.
- Income Certificate - Latest (below 2,00,000).
- Copy of the Ration card and the Aadhar Card.
- First Graduate Certificate. (BC/MBC Counselling Students only).
